Scars located on the face are notoriously difficult to deal with. The reason is that they often result in contracture, it’s not easy to apply pressure on them and it’s not easy to stretch them. This is the case with scars around the mouth. Scar contracture around the mouth can result in microstomia, lower and upper lip ectropion, as well as making the shape of the mouth become skewed. People who have microstomia will have difficulty eating or carrying out oral hygiene. Lip ectropion will make it difficult to close the mouth. In severa cases, burn patients will have difficulty controlling salivation and speech will be affected. Apart from using facial exercises and scar massage, using a mouth splint can provide prolonged stretching to the scars. This is because the foot combines flat, convex and concave surfaces. If not dealt properly, these scars can have a serious impact on ambulation. Scar growth and contracture can cause metatarsophalangeal joint hyperextension and webspace syndactyly. In severe cases, range of motion of toe joints will be affected, which may cause pain when walking or wearing shoes. In this video, Chen-Jui HSU, Occupational Therapist at Sunshine’s Taipei Rehabilitation Center will use a patient’s case to illustrate how a pressure therapy intervention can be developed for this kind of problem.

The continuous nature of the skin means that when a joint is in motion, it's not just the local skin that stretches—it's a collaborative effort involving adjacent areas. A basic concern in burn rehabilitation is the profound effect of burn scars on joint mobility. Contrary to normal skin, scars lack the ability to stretch adequately. A scar need not be directly on a joint to impede its movement. A patient may experience noticeable issues with joint mobility despite the absence of apparent scars in the surrounding skin. In the case presented here, the patient doesn't have any scars on his shoulder, yet his shoulder movement is impeded. The culprit? Uneven tension stemming from scars located elsewhere.
